Considering donation is a generous and deeply personal decision, and you are welcome here whether you are simply curious or already certain. Surro Blessings is a platform built to help you understand what donation may involve, organize your information, prepare for the steps ahead, and stay connected with the people supporting you.

To be clear and honest from the very beginning: Surro Blessings is a platform, support and coordination ecosystem. Completing an assessment is not an application guarantee. We do not guarantee acceptance, approval, matching, selection, a donation cycle, a pregnancy for anyone else, or any other outcome. Medical eligibility is determined by qualified professionals, not by this platform.

What we can offer is structure, preparation and steady communication so a complex process feels far less overwhelming.

Donor journeys differ widely. The first question in the Surro Blessings Donor Assessment asks which type of donor you are, and the rest of the assessment adapts to your answer.

• Egg Donation — The assessment adapts to egg donation, including your reasons for donating, any prior donation cycles, and readiness questions relevant to a monitored cycle.
• Sperm Donation — The same architecture with sperm-donation wording, including your reasons for donating, prior donation history, and readiness questions relevant to that pathway.

Requirements may vary depending on your location, the type of donation, the clinic or professionals involved, the recipient’s location, and any partner or agency participating in the journey. Nothing in this guide replaces the requirements set by the professionals responsible for your care.

A little preparation makes the whole process smoother. Where applicable, gather what you can and keep it nearby before you sit down.

What to have nearby

• Valid government-issued ID
• Proof of address
• Any available relevant medical information
• Any requested photos or supporting materials identified within the assessment

The Donor Assessment is how Surro Blessings gets to know you. It is designed to be completed in roughly fifteen minutes, and it collects the information our team genuinely needs in order to support your next steps.

• Contact details, background and how you prefer to be reached
• Your donor type, your reasons for donating, and any prior donation cycles
• Relevant health information, including blood type, general health, diagnoses and current contraceptive use
• About you — physical characteristics, education and a short personal introduction
• Photos, and an optional intro video if you would like to share one
• Preferences and matching — openness, future-contact expectations, boundaries and communication preferences
• Travel willingness, geographic considerations, timeline and readiness for injectable medications where applicable
• Any additional comments or supporting files you would like to include
• Authorizations and acknowledgements covering drug screening, psychiatric evaluation, medical records release and background review, plus the final terms

You will also be asked what compensation you expect. There are no right or wrong answers anywhere in the assessment — honest answers help everyone involved make better decisions.

Your Surro Blessings account is your private home base. It is where your information lives, where updates arrive, and where anything sensitive should be shared.

Every Surro Blessings member is identified by a canonical member reference. Donors use the format SB-D-####. This reference belongs to you, stays with you, and is used in member communications so you always know a message is genuinely about your journey. It becomes your stable member reference once durable member and profile records are created for you — your own reference is shown inside your account rather than on a public page.

Your dashboard is the home base for your donor journey. As your journey develops, it is intended to bring together everything in one authenticated place:

• Your donor profile and the summary shared with appropriate parties
• Your assessment status and journey progress
• Requests and to-dos, including anything our team has asked you for
• Documents you have uploaded and documents shared with you
• Communications and updates
• Appointment and follow-up information
• Opportunities, interest from partners, and matching status

Some of these capabilities are live today and others are being built as the platform grows. Where something is not yet available, our team will reach you directly and let you know.

After you submit, Surro Blessings reviews what you shared. It is completely normal for our team to come back with questions, ask for a missing detail, or request a document that could not be provided earlier.

A request for more information does not mean you have been declined. Most journeys involve at least one round of follow-up, and clarifying something early usually prevents a delay later.

After submitting, please watch your email, including your Spam or Junk folder, for updates and next steps from Surro Blessings.

Once your assessment has been submitted and your account is active, next-step support becomes far more useful, because our team can speak with you about your actual answers rather than generalities.

Your assessment includes follow-up availability — your preferred date, your earliest and latest times, and your preferred contact method. Those times are recorded as local times and interpreted against the time zone on your contact information, so we reach you when it actually suits you.

If you have a question before you begin, you are always welcome to reach out. Coordination and scheduling happen through our team rather than a public self-booking calendar.

Donation involves review by qualified professionals. Depending on the donation type, the clinic, the recipient and your location, that review may include several of the following:

• Medical screening and health-history review
• Family-health-history review
• Psychological or counseling evaluation
• Background review
• Drug screening
• Clinic-specific testing, which may include genetic and infectious-disease screening
• Additional records requested by the professionals involved

Your assessment includes authorizations covering drug screening, psychiatric evaluation, medical-records release and background review, so that these steps can proceed when the time comes. Surro Blessings does not perform clinical screening itself, cannot determine your medical eligibility, and cannot promise that any particular screening result will lead to a specific outcome.

Matching at Surro Blessings is about thoughtful compatibility, not a leaderboard. The goal is to organize potentially relevant opportunities so that the people involved can make an informed, human decision.

Opportunities may be surfaced in a few different ways:

• Surro Blessings Recommended — opportunities our team believes may align with what you have shared.
• Partner Recommended — opportunities put forward by an approved partner or agency working within the ecosystem.
• Eligible opportunities — broader opportunities you may be able to consider as the ecosystem grows.

Automated compatibility tooling is still being built. Today, review is human and deliberate. Nothing about the process guarantees that you will be matched or selected, and protected characteristics are never used as automated ranking criteria. Interest goes both ways — you are never obligated to accept an opportunity.

The Preferences & Matching section of your assessment is where you tell us what matters to you. These answers help organize potentially relevant opportunities; they do not guarantee a particular match.

• The type of donation arrangement you are open to
• Whether you would be open to future contact with a donor-conceived child once they reach adulthood
• Any boundaries or expectations you would like intended Parents to understand up front
• How often, and in what ways, you would prefer to communicate if you are matched
• The languages you are comfortable communicating in
• Willingness to travel to a clinic outside your area, and any geographic considerations
• Comfort with injectable medications where they are part of the process
• When you would like to begin, and the support you have around you
• Any families you would prefer not to be matched with, and your compensation expectations

Expectations around identity, future contact and information-sharing can vary by donor arrangement, clinic, partner, applicable law, and the preferences agreed to by the people involved. Surro Blessings cannot promise anonymity, identity disclosure, or future contact — those are settled within your specific arrangement and with the professionals guiding it.

A donor journey typically involves several independent professionals, and which ones depends on your donation type and location. You may work with some or all of the following:

• Fertility clinics and medical providers
• Agencies and approved partners
• Attorneys
• Genetic counselors
• Mental-health professionals
• Journey coordinators
• Travel and logistics support
• Insurance and financial professionals

Surro Blessings helps organize these connections and, where partners are available, can help point you toward relevant resources. We are a coordination and support platform — we do not provide licensed medical, legal, insurance or agency services ourselves. A growing resource library and partner directory is being developed to make finding the right professional easier over time.

Depending on the donation type and what the professionals involved require, your medical path may include initial appointments, laboratory testing, screening, cycle planning, medication education, monitoring appointments, and preparation for a procedure or collection.

Your assessment asks whether you have already scheduled an appointment with your OB/GYN, fertility clinic or provider for your next donor-related medical step. You can answer Yes, Not Yet, or I Need Assistance. If you have not scheduled anything yet, there is no rush — when you are ready, your provider can complete the next donor screening step and our team will follow up to help coordinate timing. If you would like help finding a provider, choose I Need Assistance and a Surro Blessings team member will reach out.

If an appointment is already scheduled, you can record the appointment type, date and time, and the provider, clinic, phone and fax details. You may also be asked when your most recent Pap smear was, as part of the standard reproductive-health picture clinics review for donors.

Some donor journeys are entirely local; others involve travel to a clinic outside your area. Your assessment asks whether you would be willing to travel if needed and whether there are geographic considerations we should know about, so nobody is surprised later.

• Where the clinic is located and how often you may need to be there
• Appointment timing and how much flexibility your schedule allows
• Transportation and local logistics
• Lodging, where it is arranged by the appropriate parties
• Advance coordination so travel is planned rather than rushed

Not every donor travels. Where travel is part of an opportunity, arrangements and any cost coverage are set by the clinic, agency or partner involved and confirmed with you in advance — they are not promised by this guide.

What actually happens at this stage depends entirely on your donation type and the protocol of the clinic caring for you. For some donor journeys this involves a monitored cycle with medications and a scheduled retrieval procedure; for others the process is considerably simpler and involves a collection appointment.

Your medical team gives you the actual instructions: what to do, when to do it, what to expect on the day, and who to contact with questions. Surro Blessings is your coordination and support platform throughout — we are not the treating medical provider, and we do not direct any part of your clinical care.

Keep your dashboard current during this stage so scheduling, follow-up availability and any requested information stay in one organized place.

Where a procedure is involved, following your provider’s discharge and follow-up guidance matters more than anything else at this stage. If something does not feel right, contact the treating provider — or emergency services where appropriate — rather than waiting.

• Attend any follow-up appointments your provider schedules
• Check your dashboard and messages for remaining tasks or requests
• Report any concerns directly to the medical professionals caring for you
• Wrap up travel or transportation details where they applied
• Reach out if you would like support resources

Surro Blessings does not provide recovery instructions. Our role is to keep coordination organized and to make sure you know where to turn.

Completing a donor journey does not end your relationship with Surro Blessings. Your account remains yours, and you are welcome to stay involved in whatever way feels right.

• Keeping your profile and contact information current
• Future donation opportunities, where appropriate and permitted by the professionals involved
• Honoring whatever future-contact expectations were agreed within your arrangement
• Sharing feedback or a testimonial, entirely at your discretion
• Referring others who may be considering their own journey
• Earn With Us, where you are eligible
• Community participation and access to resources as they expand

There is never any pressure to donate again, and nothing here implies that you are medically eligible to do so — that is always a decision made with qualified professionals.
